Below is information for new patients.
Art of Motion Physical Therapy operates as a cash pay practice, and all physical therapists are out-of-network providers. This means we DO NOT participate with insurance companies. Patients are required to make payment directly for services rendered following each session.
Should you wish to seek reimbursement through your out-of-network benefits, we are happy to provide a "superbill" upon request for you to submit to your insurance provider. However, please be aware that Art of Motion Physical Therapy is not responsible for any reimbursement or lack thereof from your insurance provider.
The current rate for a new patient physical therapy evaluation is $275 and each follow-up is $195/visit. Initial evaluations are 1-1.5hrs long to ensure adequate time for thorough assessment on the first visit and follow-up sessions are typically 1hr. We will notify you with advanced notice if these rates change.
Please complete your initial intake electronic forms at least 48 hours prior to arriving to your first visit so that your clinician can fully review this before your scheduled appointment.
There is a strict 48 hour cancellation policy in which the client will be charged $100 for not providing AT LEAST 48 hours notice for canceling or rescheduling their appointment. If a client “no shows” for their scheduled appointment without notice, they will be charged THE FULL APPOINTMENT RATE ($195-$275). For Monday appointments, cancellations must be made by the Friday preceding in order to avoid the late cancellation fee.
